Following your arrest, you were given a long, yellow form. This document is the Oregon DMV “Implied Consent Combined Report.” It is called a combined report because it has a temporary driving permit attached to the bottom of the form. The top 11 inches of the form is sent to the DMV in Salem. The form tells the DMV that you either failed a breath test, refused a breath test, refused a blood test or refused a urine test. The form also tells the DMV the duration of the suspension that the officer believes is appropriate as a matter of law. The form also is supposed to have the exact date and time of arrest. That date and time should be different from the date and time on the uniform criminal citation discussed above. The form will also contain the address on your driver license and you driver license number. You will also find the serial number of the breath test device that was employed in your case. Be aware that the address on this document will determine where the DMV will send you mail, unless you tell them otherwise. If you receive mail at some other address, you should go to your local DMV office and submit a change of address form. This large yellow form is the starting point for contesting any proposed driver license suspension. It is a very important document. Errors made on this document by the arresting officer can cause you serious, unfixable problems.
